Issue groups and occurrences

An issue group is an actionable remediation target. It may represent one or many affected DOM elements across one or many pages. The open count is therefore not the same as the total number of affected elements.

Severity

Critical and high labels help order impact, but the best first fix may also depend on journey evidence, reuse across templates or components, and the number of affected pages.

Integrity state

1. Verified: loaded-document and execution checks passed.
2. Limited: findings are usable, but failed pages or coverage gaps remain.
3. Blocked or incomplete: PixelClear could not access a trustworthy document, so no result should be used.
4. Failed: an operational or storefront error prevented completion.
